He has a sweet personality, great sense of humor and he loves, people and lots of attention.
The ideal candidate has a solid background working with young adults who have Intellectual Disabilities, Down Syndrome and Autism.
He likes attention and you will engage him in various fun activities in the home and community; therefore, experience with behavior support plans, visual schedules, PECS (Picture Exchange Communication System), and augmentative devices is essential.
Roles and responsibilities include submitting daily notes, implementing behavior support plan, sensory integration therapy, utilizing visual schedule, administering medication, assistance with bathing, toileting/toilet training, shopping and meal preparation.
He enjoys swimming, games, watching movies, arts & crafts, attending social activities and trips to the park to play and walk.
Duties and
Responsibilities:
Provide detailed daily notes on how you worked with the client on achieving the goals listed in his ISP.
This must be submitted to Robin Edwards each day by the end of your shift.
Follow behavior plan.
Meet with behavior specialist on a regular basis.
Track and record data related to behavior supports.
Submit ABC data sheet for significant behaviors Assist with bathing and grooming - This includes shampooing hair, trimming fingernails and toenail, brushing teething, flossing, Hibiclens shower or bath, and shaving facial hair.
Implement sensory diet:
whaleburger brushing, joint compression and astronaut board Utilize visual schedule throughout the day.
Food preparation - menu planning, grocery shopping and meal prep.
Follow the meal plan provided by employer.
Sit beside client while he uses good table manners, takes one bite at a time and chews thoroughly.
Jacob tends to put too much in his mouth and does not chew his food well and could easily choke.
Assist with physical exercise which includes (but not limited to) swimming, walking, and playing at the park.
Jacob needs to exercise every day, weather permitting.
Accompany client to medical and therapeutic appointments.
Assist in collecting his medical supplies in the community.
Assist with errands and shopping.
The client's ISP goals may involve the caregiver to assist in locations and activities that are not specifically listed in the job description.
Provide opportunities for the client to use his communication device throughout the day to increase functional communication:
greeting people, asking for what he wants or needs in an appropriate way, expressing emotions, and making choices.
Give prescription medications as directed by the doctor, and vitamins as instructed by the employer.
Document anything unusual with client, such as crying, prolonged teeth grinding, excessive pushiness, aggressive behavior, agitation or sleeping during the day.
Perform daily body audit and record in the daily notes.
work with client to increase daily living skills.
Allow client to fold towels, pants, and hang up his shirts.
Client may assist staff with making his bed every day.
Keep client's bedroom and bathroom clean.
This will include dusting, vacuuming, mopping, cleaning the bathtub, toilet, and bathroom floor.
Clean up kitchen after meals.
Empty dishwasher as needed.
Keep the client's closet clean and organized.
Performing the job duties for this position requires the following physical demands:
walking, reaching, some lifting (up to 50 pounds), standing, carrying, pushing, bending, stooping, kneeling, crouching, and sitting.
Other Job requirements:
patience, calm demeanor, outgoing, fun to be with, excellent verbal and written communication skills, clean driving record, dependability, punctuality, reliability, and reliable transportation with working seatbelts and doors with child lock.
A clean County/State/National background check is required.
